We provide Support Coordination at Platinum Care Direct.

Support coordination is a capacity building support to implement all supports in a participant’s plan, including informal, mainstream, community and funded supports.

•  Take care of your NDIS administration.
•  Keep track of your NDIS budget.
•  Find the right supports for you.
•  Find and connect you with providers in your community that will best support you.
•  Work with you to help you understand your plan and what supports you can access.
• Help set up service agreements with your providers.
•  Advise and guide you throughout your NDIS journey.

Once your Plan has been approved, we will work with you to determine how your funds will be spent and connect you to supports and services in the community, and help put your Plan into action.

We are committed to working closely with you, understanding your needs, and responding to you quickly and professionally. SUPPORT COORDINATION is no cost to you AND IS Funded by NDIS.

To get Support Coordination included in your NDIS plan, we recommend discussing and requesting it in your planning meeting. You have the Choice and Control over your Plan!
