REQUIREMENTS:
– Be under the age of 65
- – Be an Australian citizen
- – Permanent resident or hold a Protected Special Category Visa
- – Be living with a disability that affects your ability to perform everyday activities
– You can qualify under the NDIS early intervention requirements if you are a child under the age of 6 with a developmental delay that can be reduced by early intervention supports
Anyone meeting the NDIS eligibility is called a participant and through their eligibility, they will be able to access funded supports and an individual plan.
Supports and funding through the NDIS is available to anyone with intellectual, physical or mental health conditions who meet the access requirements.
THE PROCESS
STEP 1: Platinum Care Direct will help and support you from the beginning by ringing the NDIS to obtain access for you.
STEP 2: Once the NDIS has granted you access you will be advised of a Planning Meeting day and asked to attend.
STEP 3: Platinum Care Direct will support you at the meeting and be there with you. To get ready for your planning meeting, think about your current supports and who provides them, and what supports and services you may need to achieve your goals.
Important: You must bring along any relevant reports or assessments. It helps the planner understand the types of supports you need. During your planning meeting, make sure you discuss the goals/activities/tasks you want to achieve with your NDIS planner.
Goals/activities/tasks are a really important part of your NDIS plan and you will be asked about what you would like to achieve. The NDIS will then approve your NDIS plan. It is now up to you to choose what supports and services you feel you need to achieve your goals, who provides them and when and where they are delivered.
